Thank you so much for your donation! You will see the total owed at the bottom of this form.

Vaccination Status

We are requiring all members and guests who attend in-person services to be fully vaccinated.* Children under the age of twelve (12) and those with a documented medical exemption from vaccination are also welcome to participate in person. Those who are not vaccinated are invited to worship with us from home. Please bring your vaccination card if attending in person.

The definition of fully vaccinated at Temple Ohabei Shalom is now three doses of mRNA vaccines, or one dose of Johnson & Johnsons vaccine boosted with an mRNA vaccine. We are requiring all of our teachers and staff to get a booster shot.
